Using K.N.O.W. to Optimize Patient Learning


1️⃣ Provide General Home Health Education

Give every patient the general education during their first admission visit.

Topics include:

Risks for hospitalization

Medication safety

Materials can be printed or emailed. Collect the patient’s email address at intake.


2️⃣ Assign Patient “Homework”

Ask patients to read the education materials and complete the homework before the next nursing or therapy visit.

Some assignments may require 2–3 visits to finish due to the amount of information.


3️⃣ Review and Grade Homework

During each visit, review the patient’s homework

10-question assignments: 1 point per question

20-question assignments: 5 points per question

30-question assignment: 3.3 points per question

Discuss any missed questions and address barriers to completion (e.g., reading level, comprehension issues).


4️⃣ Customize Education for Your Patient

At admission, choose the three primary diagnoses for the patient and place their education materials in a folder.

Present additional education sections as the patient progresses.

If the reading level is too high, contact K.N.O.W. for a lower-level version—you’ll receive it within 24 hours.


5️⃣ Mastery & Certification

Once a patient scores 90–100% on a section, award a mastery certificate.

Grading Scale:

90–100% → A

80–89% → B

70–79% → C

60–69% → D

59% or below → Fail


✅ Tips for Staff

Encourage patients to involve family members if needed.

Track progress in the patient’s folder for easy reference.

Revisit unfinished assignments at the next visit and address any learning barriers.
